The Transfer Care Manager LPN/LVN supports care for Providence St Joseph Health (PSJH) patients by coordinating patient transfers. The Transfer Care Manager will be involved in taking and triaging calls, and facilitating inbound and outbound transfers for/from PSJH facilities. The transfer process encompasses communication and facilitates care along a continuum through effective resource coordination.

Required Qualifications:
Graduate from an accredited LPN/LVN school of nursing.
California Licensed Vocational Nurse upon hire.
3 years of healthcare related experience required.
