Job Description

We are seeking a compassionate and reliable Certified Nurse Aide or Home Health Aide to provide essential personal care and supportive services to clients in their homes. This role is vital in helping patients maintain their dignity, privacy, and independence while delivering high-quality care outlined in the individualized care plan under the supervision of a licensed nurse.

Please Note:
Applicants must have at least one year of hands-on experience in a similar role. An open or flexible schedule is strongly preferred to meet the needs of our diverse patient population.

Qualifications:

• High School diploma or equivalent preferred.
• Ability to read, write, and follow directions.
• Current CNA certification in good standing (if applicable).
• Proven competency in performing caregiving tasks.
• Compassionate, responsible, and dependable attitude.
• At least 1 year of experience in home health, facility-based, or private care settings.
• Flexible availability is highly preferred.

Essential Responsibilities:

• Assist with bathing, grooming, dressing, and toileting.
• Support mobility including transfers, ambulation, and prescribed exercises.
• Monitor and report vital signs and intake/output per care plan.
• Perform light housekeeping related to patient safety and comfort.
• Observe, report, and document changes in condition or environment.
• Follow the care plan and RN instructions precisely.
• Maintain timely and accurate clinical notes.
• Report any patient concerns to the supervising nurse.
• Participate in required staff meetings, orientation, and annual in-service training (minimum 12 hours).
• Adhere to HIPAA guidelines and uphold patient confidentiality.

Work Environment & Physical Demands:

• Travel to various patient homes with unique environments.
• Ability to lift, bend, and assist patients with mobility as needed.
• Clear communication skills (verbal and written).
• Exposure to moderate noise and home equipment.
