Job Description

The PRN Home Care Nurse is a practice-based Registered Nurse (RN) who directly supports Inspiritus highest risk clients authorized to receive nursing services. In collaboration with the clinical service team, the Home Care Nurse is responsible for completing comprehensive assessments for individuals with Intellectual and Developmental Disabilities (I/DD). The ideal candidate will be a team player, open to collaboration with individuals receiving services and team members. This PRN, hourly position requires prior approval from the Nursing Services Coordinator, Assistant Director, or Executive Director before assignments. Responsibilities include conducting comprehensive assessments, completing documentation, and clear communication. The role involves travel to service participants' homes and/or day programs.

Position Details:

• Monday-Friday, 8:30am - 5:00pm, as needed

• Travel to client homes and/or day programs (100%)
• Hourly: $38

Responsibilities:

• Conduct comprehensive in-person assessments of clients' physical, mental, and psychosocial needs
• Provide person-centered education to individuals and Host Home Providers
• Provide in-person training to Host Home Providers
• Document in Electronic Health Record system
• Travel to clients' homes and/or day programs to conduct assessments

Qualifications:

• Valid RN license or greater clinical license
• Valid Georgia driver's license in good standing; current auto insurance
• 2 years experience; 3-5 years with individuals with I/DD preferred
• Comprehensive nursing assessment experience
• Screening for developmental issues and psychological conditions
• Effective, respectful, and culturally competent communication
• Current or able to obtain BLS/First Aid Certification
• Current or able to obtain CPI Certification
• Knowledge of DBHDD rules and regulations - preferred
• Knowledge of Person-Centered Planning - preferred

Physical Demands:

Regularly required to talk and hear. Required to stand, walk, sit, use hands to handle objects, and reach with hands and arms. Operation of personal vehicle required. Frequently lift and move up to 10 pounds. Specific vision abilities include close vision and adjusting focus.
