We are seeking an experienced and compassionate Licensed Vocational Nurse (LVN) to join our Home Health team. The ideal candidate will have a strong clinical background and a passion for providing highquality care to patients in their homes.
Responsibilities:
Provide skilled nursing care to patients in their homes including wound care medication management and patient education
Collaborate with other healthcare professionals to develop and implement patient care plans
Assess and monitor patients health status and progress
Educate patients and their families on disease management and selfcare
Maintain accurate and detailed patient records
Comply with all applicable regulations and standards of care
Qualifications:
Current LVN license
2 years of experience in home health or a related setting
Strong clinical skills and a commitment to providing compassionate care
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Strong organizational and time management skills
