Overview:

This position requires a minimum of one year experience in a supervisory capacity in a Home Health agency.

Responsibilities:

 

  • Utilizes the quality process and best practices to improve clinical operation of the home health agency.
  • Utilizes guidelines, standards and research to support an interdisciplinary care delivery system that produces quality patient care with optimal patient outcomes. Incorporates data and information to continually improve operations.
  • Ensures quality patient care by directing the management of an interdisciplinary patient care delivery model that supports optimal patient outcomes.
  • Ensures institutional compliance with professional, regulatory, and government standards of care.
  • Monitors customer satisfaction and seeks ways to continually improve customer satisfaction/service.
  • Participates in and supports quality/performance improvement initiatives and demonstrates sound project management skills Demonstrates sound operational skills by assuring 7 day a week coverage for the home health agency, maintaining bugbear standards and effective use and appropriate utilization of all resources
  • Demonstrates effective communication skills by communicating in a direct, open, honest manner.
  • Provides staff with timely feedback
  • Effectively disseminates information about organizational mission, vision and strategic imperatives and the expectations of the staff in relation to the strategy.
  • Provides support and guidance to enhance team development by assisting team with the identification and achievement of professional growth opportunities.
  • Utilizes the performance appraisal in a fair manner and assists team with developing career goals
  • Acts as a role model for team, mentors and coaches team and peers to achieve professional career goals.
  • Creates a supportive environment that allows team to take risks to further develop. Implements action plan to improve View Point survey results and evaluates team response to changes.
  • Engages team to be involved with the agency's team engagement initiatives.
  • Makes self-visible to team by providing opportunities to meet with team, routine home visits with team ,and available office hours.
  • Handles conflict and resolution of conflict in a timely and appropriate manner
  • Provides timely follow up with team: treats team in a fair, consistent manner yet recognizes individual differences in the team Monitors team satisfaction quarterly and implement initiatives to continually improve team satisfaction. Provides routine recognition of team, both formal and informal: supports team at recognition events.
  • Maintains own clinical knowledge base and ensures staff maintain competencies outlined in the performance appraisal.
  • Creates and maintains respectful relationships throughout the Organization and external customers.

Qualifications:
  • Prior clinical experience in nursing and roles home health management.
  • Five years progressive management experience in the area of patient care and home health.
  • Previous experience as Director of a Medicare Certified Home Health Agency
  • Knowledge of Medicare Home Health certification process and Medicare Home Health Regulations
  • Demonstrates leadership abilities, including Demonstrated emotional maturity and stability.
  • Demonstrates commitment to quality patient care.
